N: The smile is one of the main facial expressions and is directly linked to self-esteem and how an individual is perceived socially. With the advancement of aesthetic dentistry, smile cosmetic surgery has become increasingly sought after, as it allows the harmonization between teeth, gums, and lips, providing natural and satisfactory results. This type of intervention may involve gingival corrections, bone remodeling, and aesthetic rehabilitations, aiming not only for beauty but also for functional and emotional balance. Therefore, understanding the procedures, indications, and impacts of smile cosmetic surgery is essential for the modern dental surgeon. OBJECTIVES: The main objective of this project is to analyze the importance of smile cosmetic surgery in orofacial harmonization and in improving patients’ self- esteem. The specific objectives are: to identify the main types of smile cosmetic surgeries; to evaluate the functional and psychological benefits achieved; and to discuss the most commonly used techniques, as well as their results and limitations. METHODOLOGY: This research will be a descriptive and qualitative literature review. Scientific articles, dissertations, and books published between 2000 and 2025 will be selected from databases such as Scielo, PubMed, and Google Scholar. The inclusion criteria will involve studies addressing aesthetic surgical techniques such as gingivoplasty, clinical crown lengthening, and lip corrections associated with the smile. After selection, the materials will be analyzed and compared in terms of techniques, results, and clinical indications, aiming to synthesize the available knowledge on the subject. RESULTS: It is expected that the study will demonstrate the relevance of smile cosmetic surgery as an effective resource for aesthetic and functional rehabilitation, contributing to patients’ physical and emotional well-being. Furthermore, it is expected to identify the safest and most widely used techniques in current dental practice, reinforcing the importance of individualized planning and multidisciplinary collaboration to achieve harmonious and long-lasting results. CONCLUSION: Esthetic smile surgery plays an important role in restoring facial balance and harmony, enhancing both function and appearance. Achieving good results requires a careful diagnosis and well- structured planning, selecting the technique that best fits each situation. The combined work of periodontics, restorative dentistry, and orthodontics makes the treatment more complete and ensures long-lasting, stable outcomes.pt_BR

CONCLUSÂO: A cirurgia estética do sorriso ajuda a devolver equilíbrio e harmonia à face, valorizando tanto a função quanto a aparência.
