Abstract: INTRODUCTION: Malocclusions, between dental maxillary atresia and osseous maxillary atresia, can be caused by several factors, such as: facial biotype, deleterious habits, mouth breathing, mandibular posture problems and early loss of teeth. It is worth pointing out that, for a long time, the treatment for this condition was performed in growing patients, thus, when it came to adult patients, rapid palatal expansion was treated surgically. However, with recent studies it was possible to show the success of maxillary expansion in adult patients, with palatal expander anchored in mini-implants (MARPE), without the need for surgical intervention. OBJECTIVE: To report the treatment of a mild case of maxillary atresia, with an aesthetic complaint of the buccal corridor, using MARPE. METHODOLOGY: The present study described the case of a seventeen-year-old male patient with maxillary atresia who was treated with MARPE. RESULTS: A class II patient, due to mandibular deficiency with previous compensatory treatment and maxillary atresia, came to the office complaining of dissatisfaction with aesthetics, mainly due to the narrow buccal corridor. Thus, the proposed treatment would involve maxillary expansion, however, due to the degree of maturation of the suture, the procedure with the conventional expander was contraindicated. Two treatment hypotheses were suggested: surgically assisted disjunction, or disjunction with MARPE. Four mini-implants were installed in the patient's palate, which supported a modified hyrax appliance, with a band on the first permanent molars. The device activation protocol was 1/4 turn per day, starting on the day of installation, for twenty days. The disjunction of the maxilla was noted, through the appearance of the interincisive diastema. CONCLUSION: The treatment of maxillary atresia in patients who are in the final stage or who have stopped pubertal growth can be performed by means of a palatal expander, anchored in mini-implants.
